Bold Improvement Goals 1. • Embed the culture of continuous learning as part of this transformation • Use a consistent language that resonates with everyone in each setting across system i.e. from primary care, the community and to wider stakeholder e.g. council, third sector and other public service staff • Ensure that engagement outside of the hospital is at the core of the BIG programme and embedded in the way of working
Bold Improvement Goals 2. • Lead by example by demonstrating values through behaviours • Support ongoing measurement and sharing of outcomes to support performance management and delivery against the agreed initiatives. Therefore communicate incremental success as it happens ensuring that there is good evidence to support this • Follow through on promises

ECG Projects (C&VUHB) • Cataract Post-op discharge service into the community re-launch in April 2016. • GP awareness of Eye Health Examination Wales • Referral guidelines produced for all GP practices and GP OOHs • Primary Care Lead Optometrist attending Locality/Cluster meetings
