  6. Who Pays for the EAP Services? • Services within the scope of the EAP are paid by Rider University • Payment for resources beyond the scope of the EAP may be covered by health insurance • BHS will assist in locating cost-effective resources when additional or follow-up services are needed

What are Your Benefits Under the EAP? • Up to 5 sessions per episode (including a full assessment and problem solving session) • Unlimited, toll-free telephonic consultation • Access to a panel of professionals nationwide • 24/7, 365 days a year access to services • Referral service and liaison with community resources including health insurance carriers • Additional work/life resources

What Can You Expect When You Call? • Connected with a Care Coordinator who will: • Thoroughly assess your needs • Guide you through the process • Assure that all of your emergent needs are met • Facilitate communication with all parties involved • Connect you to the most appropriate local resources • Regularly follow-up to ensure satisfaction

What Happens at an EAP Session? • In-depth assessment by an EAP clinician • A plan for problem resolution will be devised with the help of the clinician • Short-term counseling provided by the clinician • Problem-solving resources, outside referrals and follow-up services supplied by clinician

Can You be Assured of Confidentiality? • Shared information is held in strict confidence • EAP professionals are bound by state and federal laws prohibiting breeches in confidentiality • Information will not be released without your written consent or as required by law Note: Professionals are required by law to report if a person is a danger to themselves or others, instances of child or elder abuse, and any threats to national security.

Legal Services • Available in person and telephonically • Free initial consultation of up to 30 minutes • Discounted rate available for services beyond consultation • Addresses issues such as: divorce; custody; child support; criminal violations; wills; trust funds; estate planning Note: EAP lawyers are unable to address work-related legal issues

Financial Services • Available telephonically • Free initial consultation of up to 60 minutes • Discounted rate available for services beyond consultation • Addresses issues such as: budgeting; debt consolidation; retirement planning; financial planning; stock option planning; auto and real estate purchasing

Childcare and Eldercare Services • Assessment is conducted to understand your needs • Up-to-date national, carefully screened resources and referrals provided by a BHS specialist • Referral packets received within 12 business hours of your call which include: • Minimum of 3 referrals • Publications, brochures, and specific tip sheets related to the issue

Convenience Resources and Referrals • Up-to-date national resources and referrals for a wide range of needs including: Apartments, real estate, relocation Home/appliance repair Grocery, cleaning lawn services Pet services College / pre-college Scholarship / financial aid Gifts / tickets / flowers Entertainment and dining Meetings / events / party planning Leisure / sports activities • Referral packets received within 12 business hours of your call which include: • Minimum of 3 referrals • Brochures and specific tip sheets related to the issue

Wellness Coaching • Assigned a Wellness Coach who will: • Conduct an initial assessment • Answer general wellness questions • Provide preventative health coaching • Offer personalized health education and resources • Assist in developing a personalized plan • Assist you for up to one year for support, motivation and encouragement • Coach can assist with a variety of issues including, but not limited to: Acupuncture Diabetes Nutrition Arthritis Fitness & Exercise Natal Care Asthma Heart Health Smoking Cessation Cardiovascular Disease Massage Stress Management Chiropractic Needs Menopause Weight Management Chronic Pain Yoga
