Selecting A Pharmacy • Is the pharmacy PCAB accredited? • Does the pharmacy routinely test their compounded preparations? • Is the pharmacy familiar with the 17HP Medicaid program requirements? • Is the pharmacy USP compliant? • What does the facility "look" like? • Do they have a cleanroom and clean air equipment for sterile compounding? • Is the pharmacist available for consultation? • Do the personnel appear competent from observation and questions asked? • Is the pharmacy "clean" and "well-organized"? • Does the pharmacy have any unresolved complaints filed with the state board of pharmacy?
17 HP Important Facts Storage: Room Temperature-the vials have lots of preservative. Packaging: Should be in amber vial, requires protection from light. Properly prepared 17HP should be ready to use, no mixing, no filtering, etc. Crystallization occasionally can occur. If it does, gentle warming should re-dissolve, discard if it does not.
