Use Case 4 Care Theme: Leveraging Healthcare Registries in Care Delivery Use Case: Improving Pediatric Care and Vaccine Forecasting with Updated State Immunizations Registries • Primary Goal: To demonstrate how immunization registries are used by multiple providers in multiple states to guide a re-locating family through their child’s vaccination process. • Key Points: • Using a host of IHE profiles, this demonstration illustrates how a patient’s immunization data is shared across providers in multiple communities, and later made available for the patient to review at home. • Immunization data in two different State Immunization Registries is exchanged in an accurate and secure manner. • Immunization record integrity is maintained – patients and providers have access to real-time information. Care Theme: Leveraging Healthcare Registries in Care Delivery Use Case 4: Improving Pediatric Care and Vaccine Forecasting with Updated State Immunizations Registries Clinical Workflow: State #1 1- PCP 2- HIE 3- Immunization Registry State #2 5- PCP 4- School Nurse, PCP, & Immunization Registry A 4 year-old patient visits his Primary Care Physician (PCP) for a routine checkup, during which time the PCP administers new vaccinations in accordance with the recommendations received from the local Health Information Exchange (HIE). Once administered, a record of the vaccination event is submitted back to the HIE whose Public Health Gateway (PHG) in turn submits the event to the State’s Immunization Registry. The State Immunization Registry merges the new immunization data into the existing history for the child, then sends a comprehensive immunization history, as well as a vaccination forecast, to the HIE. The child’s family then moves to a new State, and learns during the school registration process that a new vaccination is needed before the child can enter school. The childvisits his new PCPwho, leveraging the immunization history recorded in the child’s original State, confirms the school’s assessment and administers the missing vaccination. The following year, the parents suspect that additional vaccinations are needed for the new school year, and bring the child to the PCP for review. The PCP, leveraging immunization content that’s in both (1) the child’s original State immunization registry, and (2) the new State’s immunization registry, realizes that a new vaccination is in fact missing, and administers the new vaccination during the visit.
