Case consultation • 45 years / Lady • Severe pain low back – 3 months • Bed ridden – 2 months • Flexion Contracture of B/L knee • Draining sinus – Left iliac fossa and left knee • On ATT for last 1 month
Surgery • Decompression and Stabilization • Anteriorly – Spinal mesh • Posteriorly – Pedicle screws • Tissues sent for biopsy • HPE - Tuberculosis • Post-op status • Significant pain relief • Knee contracture improved • Started ambulation with brace support
Follow up • Very happy and ambulatory – 3 months • At 6 months • Severe pain – difficulty turning posture in bed • Fluctuant swelling on the back • Explored and debrided – Sterile pus • Pain relieved – happy again • At 9 months • Very severe pain • Difficulty lying on the bed • No swelling on the back • Neurological status normal
