  3. Mishap/Near-Miss/Unsafe Reporting Procedures • Mishap - is any unplanned or unexpected event . • Near-Miss - is any situation which allowed to go unchecked or uncorrected has the potential to cause a mishap. • Unsafe Reporting Procedures - are the responsibility of all DOD personnel.

  4. Pre-Mishap Plan • Contingency plan to be implemented in case of a mishap. • All classrooms and laboratories shall have a pre-mishap plan posted. • Shall be reviewed monthly. • TSO shall be designated for each course and is responsible for documenting walk-throughs, and drills.

  5. Lessons Learned • The most significant Lesson Learned for the RSSTP is the Mirecki Incident. • This course is a direct result of that incident. • The primary objective of RSSI is to educate Instructor Candidates in the proper use of the curricula.

  6. Lessons Learned for RSSI • Minor injuries have resulted when Instructor Candidates were in poor physical conditions. • Suspected water aspirations have occurred as a result of students pushing themselves too hard. • In the event of an in-water emergency students will follow instructions from staff.
