  2. Objectives • Add a new patient to patient MedTrak database

  3. Key Concepts • Meaningful use • Birth date • Gender • Preferred language • Race • Ethnicity • Audit log

  4. Adding a patient • After logging in to MedTrak Main Menu appears

  5. Patient Registration button

  6. List Processor • List processor screens in MedTrak present contents of database of records.

  7. Add Patient Button If patient does not give SS# insert 999-99-9999 Click Submit when finished

  8. Patient Demographics Screen • If the patients social security number is already in database • Patient’s demographic information will be displayed for review • Otherwise, only social security field is pre-populated

  9. Patient Registration Screen • Red asterisks appear next to fields that are required • If a required field is not completed message appears in red below the date and time at the top of screen and the cursor is placed next to field that needs information

  10. Required fields • Meaningful Use Core Objective 7: • Birth date • Gender • Preferred language • Race • Ethnicity

  11. Entering information • Use patients registration form • Use appropriate punctuation and capitalization • EXAMPLE: 258 West Olive Street • NOT: 258 west olive street • OR: 258 WEST OLIVE STREET

  12. Companies screen • If simply adding a new patient companies screen is not needed • When companies screen appears click EXIT SCREEN BUTTON • Patients screen appears again, with recently added patient in the patient database

  14. Patient demographics audit log • Enables users to review who made what changes • Patient’s demographics • What changes were made • When the changes were made • Recording this health care data in an audit log is part of Meaningful Use Core Objective 15

  15. Place cursor next to command field next to patients name & click More functions

  20. Viewing your report Click EXIT SCREEN BUTTON on PATIENT LOG PATIENTS SCREEN APPEARS Click VIEW PRINTS REPORTS Print

  21. PATIENT LOG Click Printer Icon at top left To SAVE PATIENT LOG Click Diskette Button at the top left. Close window
