Long Term Conditions • Long term conditions = not cured, but can be controlled or lived with. • Include diabetes, high blood pressure, mental health, heart disease, arthritis • By the age of 60, half of adults have a long term condition • Lead to 80% of GP consultations & two thirds of hospital admissions in adults

Example: Cancer • Survival rates for most of the 20 common cancers improving • 50% of cancer patients survive beyond 5 years • Priorities for action to reduce deaths & impact: • Reduce smoking, increase normal weight & physical activity, fruit & veg consumption, use of sun screens • Increase uptake of cancer screening • Raise local awareness of cancer symptoms • Commission services in line with NICE & national guidance

How could it make the biggest impact? • What is the role of the voluntary sector in Ealing raising awareness about (choose one/two): • increasing moderate physical activity to 5 times per week for 30 minutes • stopping smoking • benefits of cancer screening • cancer red flag symptoms (blood in poo, urine, lumps etc) • carer support • support for survivors • end of life care What do you need from NHS & LA partners to enable a)-g)? How could voluntary sector impact be identified?
