The Scope of the Problem • Patient harm inflicted by health care facilities is a global problem • It is estimated that 10% of all inpatient visits result in unintended harm in developed countries • On average, 2.2 million individuals from our Region are harmed every year in health care facilities • Potentially preventable in 50-80% of cases
Introduction Why medical errors occur • Mostly system errors rather than individual • Poor infrastructure • Lack of standards and protocols • Poor technical design of medical equipment • Weak safety culture: Denial & Blame • Ineffective Communication
The response of WHO • World Alliance for PS launched in 2005, in response to WHA Resolution urging Member States to pay the closest possible attention to PS

Steps towards Patient Safety in EMR Research
Research • Initiated a rapid assessment questionnaire to determine PS status in countries • PS research project: To determine the prevalence of adverse events in health care facilities by medical record review using a validated tool

Research for determination of adverse events in EMR Morocco Sudan Egypt Jordan
15,000 records have been reviewed and analyzed from 27 hospitals in the Region following these steps:
Tools introduced in 2007 during the 3rd research meeting Include observation, staff interview and focus groups Pilot testing already initiated in Jordan and Tunisia Research • Initiated a rapid assessment questionnaire to determine PS status in countries • PS research project (EMRO/AFRO) update • Introduction of alternative tools for AE determination in data-poor environments

The Patient Safety Friendly Hospital Initiative PRINCIPLE Project to implement safe practices in health care facilities by assessing adherence to PS guidelines developed by WHO Incorporates all PS concepts and programmes, e.g. PFPS, CCSC, SSSL, research etc. PROGRESS Developed a Regional proposal Circulated proposal to WRs and country offices Initiated guideline development
