Federal Overview • Federal demonstration created by the Deficit Reduction Act and offered through the Centers for Medicare and Medicaid Services • Partnership between Maryland’s Department of Health and Mental Hygiene (State Medicaid Agency) and CMS • 5 year demonstration designed to: • Assist states in rebalancing long-term care systems • Increase the use of Home- and Community-Based Services (HCBS) • Remove barriers to receiving services in the community
Maryland Transitions • To access MFP funds, states must transition individuals receiving Medicaid services from “qualified institutions” to “qualified residences” • Individual must have lived for 6 months in institution • Individual must have 1 prior month of Medicaid eligibility PopulationTransitions • Older Adults 1361 • Adults with Physical Disabilities 371 • Adults with Developmental Disabilities 250 • Other Disabilities (Brain Injury, Mental Illness) 12 Total Transitions 1994
Qualified Residences • A home owned or leased by the individual or the individual's family member; • An apartment with an individual lease, with lockable access and egress, and which includes living, sleeping, bathing, and cooking areas over which the individual or the individual's family has domain and control; • A residence, in a community-based residential setting, in which no more than 4 unrelated individuals reside.

July 2008 Updates • Peer Outreach in NFs • RFP IN procurement process, to be posted in August, award anticipated in November • Transition Center • RFP is at DBM; anticipate posting in August, award in November • Ongoing Peer Support • RFP being written, anticipate posting in November • New Waiver Services • Incorporated into regulations for the Living at Home and Older Adults waiver in June
July 2008 Updates • Peer and Family Mentoring in SRCs • MOU with the DD Council to write and post the family mentoring RFP in process • Anticipate posting in September, award in November • Self-Advocacy Network contract enhanced in June • DDA Staff • New Transition Coordinator began July 1st, • Community Placement Specialists begin by August 1st • Behavioral Health Workgroup • Continues to meet and explore the integration of somatic and mental health services
