Child Care Forms • Child Care Eligibility Certification Report – 2050 • Auth for Child Care Enrollment – Selected Referral – 2450 • Auth for Child Care Enrollment – Selected Provider – 2450 • Child Care Notification of Denial or Reduction – 2052/1071
2050 –Child Care Eligibility Certification Report • Enter TWIST ID of a Child Care Customer • Go to the Child Care Program Detail for which you want to print the report • Click on the Printer Icon to view available forms • Select “Child Care Eligibility Certification Report – 2050”
2050 –Child Care Eligibility Certification Report • Form is displayed for review • Items 1 – 7 display data from information in Intake Common
2050 –Child Care Eligibility Certification Report • Items 8 – 14 display data from the Family and Income tabs on the Child Care Program Detail
2050 – Child Care Eligibility Certification Report • Maximum Allowable Income is displayed based on information from the eligibility code card

2450 – Authorization For Child Care Enrollment - Selected Referral • Go to the specific Program Detail referral tab • Select a child from the dropdown box • Highlight a specific child care referral
2450 – Authorization For Child Care Enrollment - Selected Referral • Click on the Printer Icon to view available forms • Select “Auth for Child Care Enrollment – Selected Referral – 2450”

2052/1071– Child Care Notification of Denial or Reduction • Enter TWIST ID of a Child Care Customer • Go to the Child Care Eligibility Menu Item • Click on the Printer Icon to view available forms • Select “Child Care Notification of Denial or Reduction – 2052”
2052/1071– Child Care Notification of Denial or Reduction • Review Form • Child Care Contractor and Comments will have to be completed after the form is printed • Click the Print Button to print the form
Adding text to Forms • Hand Written (Typed) • Stamp with Contractor name/address with rubber stamp • Entered Electronically • A method of entering input into a digital form such as Microsoft’s Digital Imaging tools (included with Microsoft Office) may be used. • One such tool is Microsoft Office Live Meeting Document Writer which produces a MDI form that can be edited prior to printing.
Adding text to Forms – One method • Entered Electronically (cont.) • When selecting a printer, select Microsoft Office Live Meeting Document Writer if available • Must use office 2003 or 2007 to use the printer • Can be installed from original Microsoft Office installation media • Install the Document Imaging option
Adding text to Forms • Click the Print Button to print the form • Select Microsoft Office Live Meeting Document Writer • Click the Print Button which automatically creates the *.mdi file
Adding text to Forms • Select the path to save the document • Type in the file name, do not change the suffix “.mdi” • Click the Save button
