3. Poster Contest Entrant Media Release Form Our school is having a Digital Citizenship Poster Contest and your child is eligible to enter. When the winners are selected, we would like the opportunity to publicize his/her work both in print and online. With your permission, your child’s full name, the name of his/her school, and an image of his/her work may be published in the press, used for commercial purposes, and/or displayed on the internet. This is a wonderful opportunity for your child to receive recognition for his/her work. A signed release form is required for every student whose name/image/likeness is printed or posted online. • If you choose not to sign this release, we will simply list the name of the school and an image of your child’s work without mentioning his/her name. • If you would like your child’s work to be credited, please fill out the form below. • I_____________________________________ (parent’s name) give permission for my child’s name ___________________________(child’s name), to be published in connection with his/her Digital Citizenship Poster Contest entry.
