  2. Inpatient Mortality Index BMC wants to achieve a score of .77 or lower. FM has always been lower than that. Source: UHC, Patient Outcomes Report

  3. Daily Workload by Care Team (Census Counted At 8AM) From month to month, FM1 and FM 2 have a similar average number of patients Source (both graphs): SDK Combined Census Report and confirmed with EPIC census on weekdays

  4. Weekly Average Discharge Time The gap in average discharge time between FM, BMC, and comparable floors is starting to close Source: Weekly hospital reports (From Jonathan Bunker)

  5. Monthly Total Number of Patients Discharged by Care Team FM-HN continues to maintain a constant volume about 8-9 discharges per day. Note: Total Number of Patients Discharged = Inpatient + Obs Source: EPIC, DOM Discharges by Month report

  6. Monthly Percentages of Discharges in Obs Status by care team The % of patients discharged in OBS satatus decreased for FM 1 and CCA but increased for FM 2 Source: EPIC, Department of Medicine Admissions and DOM Discharges by Month Report

  7. ALOS of Inpatient Discharges by Care Team (In Days) Based on Mean- Since may, the ALOS for FM 1 and CCA increased and stayed the same for FM 2. Based on Median- Since May the ALOS for all three teams increased. Source: EPIC (From DOM Discharges by Month report) Note: The ALOS goal came from BMC Inpatient Data Dashboard (Medicare ALOS)

  8. Alos of Observation Discharges by Care Team (In Hours) Based on both the mean- the ALOS for FM 1 and 2 increased and decreased for CCA since Mar.
