Tx and update EHR Scotty asks the ER Doc to send a copy of his discharge summary to both his PCP back home and the Doctor he will be following up with back at school. Scotty only knows the name of the Doc he will see in Sonoma. What is an ER Doc to do??? He’s got Scotty’s PCP’s Direct Address already but not his doc back at school.

The PCP’s Direct Address • For the purposes of the demo, we have elected to simulate that the local Provider Directory at SDBC (i.e., Mirth’s PD) already has a direct address and digital cert for Scotty’s PCP so that we don’t have to go through the retrieval demonstration process twice.
ER Doc adds Scotty’s PCP’s Direct Address to the Message and sends Dr. Petty SDBC Transforms content from EHR to the Transitions of Care S&I Framework standard output for Discharge Summaries.
