Institute for Clinical Systems Improvement • Founded in 1993. • Collaboration of multiple healthcare organizations. • An objective voice dedicated to championing health care quality and to helping its members accelerate the implementation of best clinical practices for their patients. • Patient Safety has been one of the focuses.
Patient Safety Committees • Committees exist in both our Hospital and Medical Group environments. • Focus has been on: • Creating a Culture of Safety • Improved Reporting • Staff Education • Partnering with our Patients • Medication Safety • Equipment Safety • Safety Practices
Safest in America • Collaborative of the major healthcare providers and ICSI. • Goal is to reduce harm to patients resulting from errors. • Medication Practices • Correct Site Surgery • Exchange of “best practices” for patient safety
Pharmacy Strategy Project • Enterprise-wide initiative developing a pharmacy plan focused on quality of care, service to our patients and members, and the optimization of healthcare costs. • Seven Major Goals developed, one goal is: Providers have access to electronic prescription writing capabilities.
Pursuing Perfection • Robert Wood Johnson Foundation (RWJ) with the Institute for Healthcare Improvement (IHI) issued a call for proposal, “Pursuing Perfection: Raising the Bar for Health Care Performance” in February, 2001. • “Crossing the Quality Chasm” published by the Institute of Medicine in early 2001 is a call for a sweeping redesign of health care. • “Crossing the Quality Chasm” sets forth six aims for improving today’s health care system.
Pursuing Perfection • Pursuing Perfection uses the six aims as the dimensions around which to organize improvement. • Patient-centered • Timely • Effective • Efficient • Safe • Equitable
Pursuing Perfection • The goal of this project is intended to help hospital and physician organizations dramatically improve patient outcomes by rethinking major processes in terms of perfect care. • Health information availability, best science, care alerts and reminders are considered as infrastructure necessary to the entire project. • Although we applied and received this grant for a Medical Group project, it has become an enterprise-wide initiative.
Leapfrog Patient Safety • Computerized Drug Orders • We agree that CPOE is important • Implemented Physician Order Entry in our hospital in the early 1980’s on the Eclipsys TDS 7000 product. • Culture and technology were not in sync. • If fully in use today for all pharmacy orders, we would still not meet the Leapfrog functionality requirements for override documentation. • Clinical System planning efforts scheduled to begin in September 2002 . This will include CPOE, with a goal to start the implementation in 2004. • Currently rolling out the Electronic Medical Record in our Medical Group Clinics. (2 sites live)
Leapfrog Patient Safety • Other Pharmacy Initiatives we believe will support Patient Safety and CPOE: • Pharmacy Information System implementation in the hospital is our 2002 focus. • Medication dispensing system replacement project in progress for 2002. • Look at medication administration bar-coding as a potential 2003 project.
