Example • A- Mom was worried she was gaining to much weight. Her weight gain is right on track w/ her grid. • D – Mom mentions how much she loves sushi. She also tells you that she is thinking about BF but is not sure if she can because she has to go back to work as a teacher.
Example • “I’m really trying to do right, and get back in school. I’ve been taking a ton of classes and its been kind of hard keeping up with the homework.” • “No, I don’t really have any questions about that. But, do you know where I can sign him up for Karate classes? I’ve really wanted to do that.”

Example • Assessment info: You have just completed a certification on an IPN baby. • You found no A, B , C or E codes. • When you ask about D you learn that baby is mostly nursing but mom would like to fully nurse. She was worried baby wasn’t getting enough. Currently she takes about 12 oz (3, 4 oz bottles/day) of Similac Advance and the rest is breastmilk. She was keeping a bottle out for more than 2 hours.
Example Continued • Nutrition Education: You offer mom information about breastfeeding and how to tell if baby is getting enough. You also ask if you can share info about formula storage. She tells you she will stop holding the formula out and that she would like to see if she can get off formula completely. • Food Package/Next Steps: She tells you she would feel more comfortable with 3 cans and would like to talk to the PC. You schedule her to come back next month for her Hgb check and give her 3 cans.
