1. Welcome to Lehigh Valley Health Network The files included are orientation materials that must be read and completed prior to any experience at Lehigh Valley Health Network. You will need Adobe Acrobat or Reader, and Adobe Flash Player to access the files.

3. New Instructors - All of the documents must be completed and sent to the Office of Student Affairs at least two weeks priorto you arriving at Lehigh Valley Health Network for your experience. • Recurring Instructors – The Documentation of Orientation form must be completed and submitted to the Office of Student Affairs annually.

4. Current Employee Requirements • If the instructor is a current LVHN employee, please complete an Orientation Exemption Form Only.

5. Orientation Requirements • Please start with the Documentation of Orientation file. Open and immediately print this file. This file contains: • Documentation of Education form • General Safety Quiz • HIPAA Quiz • Acknowledgment of Confidentiality

6. New Instructors to LVHN • New instructors at LVHN are required to complete all required Health and Criminal Clearances. • If your school has completed your health and criminal clearance please have your school complete the School Documentation Form in lieu of criminal and health report submission. • If your school has NOT completedyour health and criminal clearances please submit the Health Certificate Form and All Required Criminal Clearances to The Office of Student Affairs.

7. Unit Specific Orientation • Contact with the unit Director, PCC or PCS must be made at least 2 weeks prior to each clinical session to discuss course objectives, unit requirements, unit initiatives, unit orientation etc. • If required orientation documentation is not submitted and arrangements with the unit Director, PCC, or PCS have not been made at least two weeks prior, the clinical start date will be delayed until all requirements are complete.  

8. During your clinical experience at Lehigh Valley Health Network, you mustwear your Identification Badge at all times! If you do not have an ID badge, please contact LVHN Office of Student Affairs at 610-402-2482 immediately.
