Bulletin Objective • Surveillance is information for action • Quick dissemination of information to all stakeholders is one of key objectives of surveillance. • The focus is on providing quick facts on level and trends of the epidemic at the state and district level. • All data in this bulletin are provisional and subject to future revision.

ANC HIV Sentinel Surveillance 2010 HIV Sentinel Surveillance is crucial to understanding of HIV epidemic dynamics among different populations in different settings and subsequently to estimate the prevalence and HIV burden at the district, state and national level. In Andhra Pradesh HSS was initiated in 1998 at 6 sites. Significant up-scale of HSS sites among different groups, sector and geography

ANC HSS Methodology • Unlinked Anonymous Testing (UAT) without informed consent • Consecutive sampling • Inclusion criteria • Age 15-49 years • Pregnant women attending the antenatal clinic for the first time during the surveillance period. • Exclusion criteria • Already included once in the current round of surveillance • Pregnant women aged <14 years or > 50 years

  8. HIV Prevalence among ANC : Current Level (2010) • Median Prevalence of all sites is 0.50% • Median prevalence of consistent sites is 0.75% • Mean Prevalence of all sites is 0.77% (95% CI 0.66-0.87) • Mean prevalence of consistent sites is 0.90% (95% CI 0.76 – 1.04)

Summary of Findings • The median prevalence recorded during HSS 2010 among ANC clients was 0.50% while mean prevalence recorded was 0.77% (95% Confidence Interval 0.66- 0.87). • The median prevalence recorded at 44 consistent sites was 0.75% while mean prevalence of these sites was 0.90% (95% Confidence Interval 0.76- 1.04). • Mean prevalence at urban sites was 1.09% (95% CI 0.88-1.30), while it was 0.70% at rural sites (95% CI 0.52-0.88). Lowest mean positivity of 0.44% was recorded at sites belonging to CHC-PHC category (95% CI 0.26-0.63). The mean positivity at private sector sites was 0.46% (95% CI 0.19-0.73). • The prevalence of less than <1% among ANC clients has been recorded for the first time by HSS in state. The statistically significant decline is linear in nature.

Summary of Findings (Contd..) • Except for six districts, all districts have recorded a mean prevalence of less than 1%. • Highest mean prevalence was recorded at Kadappa (2.25%) followed by Karimnagar, (1.25%), East Godavari (1.25%) and Guntur (1.08%). Krishna and Mahboobnagar recorded a mean prevalence of 1%. • Five Districts (Hyderabad, Medak, Nizamabad, Warangal and West Godavari) recorded a mean prevalence in range of >0.75 - <1% • Nine districts (Adilabad, Anantpur, Chittoor, Kurnool, Rangareddy, Nalgonda, Khammam, Vishakhapatnam and Srikakulum) recorded mean prevalence in range of > 0.50% to <0.75%.

  25. Summary of Findings (Contd..) Lowest mean prevalence of 0.25% was recorded in district of Prakasham. Other districts which recorded less than 0.5% mean prevalence were Vizianagram and Nellore. Districts of Kadappa, Karimnagar and Visakhapatnam with increasing trend emerges as new challenges for HIV prevention efforts. Districts of Rangareddy and Srikakulum are fit to move out of Category “A”. Additional years of data are required before it can be established whether the less than 1% prevalence among ANC clients is temporary or long-term.
