Intensive Evaluation Mission/Purpose of Program: In the intensive evaluation, the staff to client ratio is increased to provide even more comprehensive services to individuals with significant barriers to employment. One assistive technologist and one vocational evaluator work together to evaluate one participant for five days. Goals of the intensive evaluation are to address performance enhancement, independence, and quality of life. The infusion of assistive technology often allows the participant to explore options that were previously not considered. All possible technology needs of the participant are considered including activities of daily living, mobility, adaptive transportation, computer access, worksite accommodations, and environmental controls. Goal: To increase an individual’s performance, independence, and quality of life is our goal. This may include vocational and non-vocational endeavors.

Intensive Evaluation Primary Population Served Referral Source (s) Division of Vocational Rehabilitation School Districts Veteran’s Administration Managed Care Organizations Private Referrals Insurance Companies Businesses • Individuals with disabilities or work related injuries. • Individuals with severe or multiple disabilities that may not typically be referred for vocational evaluations. • Program is open to all ages, cognitive levels, and physical abilities.

  4. Intensive Evaluation • Length of eval: 5 consecutive days • Location: Intensive Evaluations are completed in-house at SVRI located on the UW-Stout campus. In addition to the main evaluation lab, there is access to the SVRI sensory lab, seating and mobility lab, independent living lab, and the fabrication lab. • Costs: The Intensive Evaluation is $3902.00, room and board is $248.50, attendant care billed at $16.00/hr, and nursing care billed at $25.00/hr.

  5. Intensive Evaluation • Staff involved: One Certified Vocational Evaluator and One Assistive Technology Practitioner. • Assessment tools used: • Psychometric tests • Work samples • Situational assessments • Career exploration

  6. Intensive Evaluation • Collaborative effort: The CVE primarily administers psychometric tests, work samples, and career exploration activities. The ATP provides accommodations to the evaluation site as well as to the tools used by the evaluation staff. The entire 5-days is a collaborative effort between the CVE and ATP in order to provide comprehensive services to the consumer.

  7. Case Study David 20-year old, senior in HS WI DVR referral Spina Bifida, Congestive Heart Failure, and a learning disability. Receives Social Security benefits Interested in supplementing SS benefits with employment.

Case Study • David stayed in the residence hall throughout the 5-day evaluation. • He chose to have his father stay with him to provide personal cares including catheterization, transfers, and assisting with meals. • David participated in psychometric testing, work samples, career exploration, and a situational assessment; all with modifications.
