Risikofaktor Myokardischämie • Erhöhung des Risikos perioperativer Komplikationen: - VT und Herzinsuffizienz um Faktor 2,8 - Koronarsyndrom, AMI, Herztod um Faktor 9,2 • Inzidenz 0,1- 15 % (vor allem gefäßchir. Pat.) • 80 % verlaufen zunächst stumm • 25 % prä/intraoperativ und 50% postoperativ • bei Kombination von Alter > 65J, Hypertonie, Hypercholesterinämie, Diabetes, Nikotinabusus beträgt das Risiko von kardialen Komplikationen 34 % Dr.Kai-Uwe Döbel

American College of Cardiology/American Heart Association perioperative assessment guidelines for noncardiac surgery reduces cardiologic resource utilization preserving a favourable clinical outcome • Guidelines implementation reduced preoperative cardiologic consultations by 21% (P < 0.001) and preoperative non-invasive diagnostic testing by 11% (P = 0.01), and increased utilization of preoperative beta-blockers by 13% (P = 0.01).
