MemberResponsibility • Timely submission of “Proof of Relationship” • Medical, Dental, & Vision are taken pre-tax, elections can not be changed unless there is a Qualifying Event • Timely notification of Life Status changes/ Qualifying events (31 days) • No Double Coverage

Term Life Insurance Basic & Voluntary Life • The Hartford • Basic Life Insurance is 140% of base annual salary up to $50,000 paid for by the City. • No evidence of insurability required for coverage up to $250,000 • Maximum coverage amount 7 times annual salary not to exceed $500,000 • EOI required for amount over $250K
Dependent Life Insurance • Spouse coverage • Coverage can not exceed employee’s coverage effective 7/1/2013 • Dependent Children • Covered up to a maximum of $10,000 • Employee must have coverage effective 7/1/2013 • Enhancements: Funeral Planning, Concierge Services, Travel Assistance and ID Theft Protection Services. • Plan Overview of pages 28 & 29
