Near collision – either positive or avoiding action or luck prevented a collision Air proximity – collision did not occur, but aircraft were close enough to cause alarm Loss of separation – loss of required separation; runway separation, required terrain separation (IFR below MSA), with IFR aircraft in controlled airspace
Note: more dual hours flown per year – solo are around 60-70% of dual hours

An SMS is designed and implemented to: • Develop and improve the safety culture within the organisation • Manage risks within the organisation – with particular focus on risks which impact on safety • Provide for ongoing monitoring and assessment of safety performance • Make continual improvements
The culture shift… • Move from intuitive environment to planned management of risk • Proactive approach • Identify hazards – prioritise • Some hazards might not become a risk
