  3. 2012 Action Plan • Goals: • Strengthen promotion and partner engagement across all NDEP activities. • “Mapping” of NDEP resources in order to make it easier for all partners to find relevant materials across all of NDEP’s platforms. • Expand evaluation of NDEP efforts. • Exploration of Electric Health Records to share NDEP resources.

Task Groups • Guiding Principles Task Group • Goal: Revise Guiding Principles for Health Care Professionals with a set of common principles supported by major Health Care Professional organizations to help them manage their patients.   • Promotions Task Group • Goal: Assess NDEP’s offerings and create a plan to expand NDEP’s brand awareness, enhance NDEP’s reach of diabetes/health messages, increase access to NDEP’s content, further participation & engagement with audiences.

Task Groups (cont’d) • BetterDiabetesCare (BDC) Website Task Group Goal:Guide an update and integration of important information on systems of care into NDEP’s tool. • Support for Behavior Change Initiative Phase III Task Group Goal: Review and update new resources for Diabetes HealthSense and identify gaps and next steps.

Task Groups (cont’d) • New Beginnings Task Group Goal: Develop supporting materials using a storytelling approach to include in the New Beginnings discussion guide for health and diabetes educators to use with African American families and replace materials dependent on The Debilitator movie. • DiabetesAtWork Website Task Group Goal: Package the various elements of the DiabetesAtWork site to increase use by public and private partners in the business and health strategy world.

Task Groups (cont’d) • Evaluation Plan Task Group Goal: Develop an overarching monitoring and evaluation plan for NDEP and its partners and to develop a plan for monitoring and evaluating the partnership transition. • PPOD Primer Task Group Goal: Update, revise expand, and reformat the existing Primer content and include additional resources, such as the patient checklist developed by PPOD and the HCP WGs and consumer pieces. • Migrant and Seasonal Farmworkers Task Group Goal: Conduct an environmental scan and needs assessment related to cardiovascular disease, prediabetes, and diabetes health education needs of migrant and seasonal farm workers (MSFW).

  8. Call for Stakeholder Group Involvement • Health Care Professional GamePlan - To be updated and reviewed with updated information around primary prevention. • Articles for School Nurse News and National • Association of School Nurses • - Seeking volunteers to write articles

Call for Stakeholder Group Involvement • Assist NDEP in pre-testing NDEP materials with target audiences • NDEP has been conducting a review of its most popular publications for health literacy/plain language • principles, including: • Content • Literacy Demand • Health Numeracy • Graphics and Visuals/ Layout and Design • Learning Stimulation, Interaction and Motivation • Cultural Appropriateness

Call for Stakeholder Group Involvement • Stakeholder Feedback to Branding Survey – Thank you for your Input! • Goal of activity is to: • Promote better recognition of NDEP and its resources • Refresh NDEP’s logo • Standardize design template and color palette forpublications • In process of Reviewing Stakeholder Feedback – More to Come!
