Who are we? • Over 50 active partners from government agencies, non-profit organizations, college districts, medical and therapy practices, residential facilities, and businesses • Working under a grant from the Archstone Foundation

Who are we? In 2005, the agencies and organizations in Orange County that work with the older adult populations recognized the need to address this health concern. A “Down with Falls Forum” was held in Orange County in July 2005. As a result, several government agencies, non-profit organizations, college districts, medical and therapy practices, residential facilities, and businesses made a commitment to the development of a Down with Falls Coalition to serve the needs of older adults in Orange County. In order to develop the Down with Falls Coalition, Rebuilding Together Orange County and County of Orange Office on Aging received an 18 month coalition-building grant from the Archstone Foundation. The overarching mission of the Down with Falls Coalition is to prepare strategically, financially, and cooperatively to provide awareness and accessibility to the fall prevention components of balance and mobility training, medical management, and home assessment and modification to the neediest population segments of the county. The Coalition’s objectives are to: 1) create a strong, committed, and sustainable Fall Prevention Coalition; 2) conduct a countywide fall prevention needs assessment; 3) create a 3-5 year strategic plan; and 4) revitalize, enhance, and evaluate the Coalition at a community forum in 2007. The coalition members have completed the needs assessment collection. A total of 838 surveys were collected from Orange County older adults! The strategic planning process began with a lively meeting where coalition members offered their input for proposed activities for the coalition to undertake over the next year or two. Development of the strategic plan will continue over the next several months. 5 Primary Goals • Goal I: To increase in size and diversity and become a recognized entity in the community. • Goal II: To become a “household brand” for fall prevention programs, services, and resources. • Goal III: To become a comprehensive clearinghouse for fall prevention knowledge and resources.
Goal IV: To increase awareness and knowledge of strategies to lower the risk of falls. • Goal V: To advocate for changes to the built environment to lower the risk of falls in Orange County.
Major Activities • Maintain and increase active coalition membership. • Establish regular meeting times and institute working subcommittees. • Seek out, research, and apply for funding opportunities. • Develop and maintain a database of fall prevention resources.
Major Activities (cont.) • Produce and distribute fall prevention materials. • Resource and referral guidebook • Fall Prevention kit for older adult use • Education materials, fall prevention checklists, safety tips, assessment tools, etc. • Develop education programs for service providers: • “Educate the Educator” program • Speakers Bureau • Physician offices • Transportation and mobility services
Major Activities (cont.) • Conduct fall prevention workshops for older adults. • Identify and prioritize 5 locations in Orange County in need of safety improvements. • Educate Orange County elected officials on fall prevention issues.

Purpose Develop a packet of older adult “friendly” fall prevention information that can be distributed to persons at risk of falling in Orange County.
Kit Contents (Proposed) • Fall Risk Self Assessment • Fall Prevention Checklist • Health Record Booklet (including fall specific information) • Exercises to Improve Balance (Booklet) • Center For Disease Control Pamphlet (What You Can Do To Prevent Falls) • Community Resources reference card with DWF information contact number • Gift with DWF logo (shoe horn, night light, etc.)
DWF Health Record Book Includes Fall Prevention Specific Reminders: • Talk to your doctor about your balance and falls • Update your record when changes occur • Bring your health record to all health related appointments • Manage your health and medications to reduce your risk for falls
DWF Health Record Book Includes Tips To Prevent Falls: • Review the effects of medications and alcohol use on balance • Engage in regular balance and strength building activities • Identify and correct falling, tripping, and slipping hazards
DWF Health Record Book Includes space for recording: • Medications (prescription, over the counter, herbals, dietary supplements) • History of Falls • Balance Related Concerns
Evaluation & Mass Production of Kit • Feedback from DWF Forum attendees • Focus groups with older adult groups in the community (Fall 2007) • Revise kit based on feedback • Mass production of kit for distribution to at risk older adults in Orange County at a later date
