Transition Planning Process • Transition planning begins at age 14 but can occur sooner • All team members play an important role • Checklists • Benefits • Patient becomes more knowledgeable about medical condition and learns new skills • Patient becomes more independent, confident, and self-sufficient • Adult providers have expertise in caring for adults

  5. Managing Health Care Needs • Symptom management • Daily care • Learning names and dosages of medications • Medication refills • Scheduling own medical appointments • Meeting with health care providers independently • Identifying adult providers (PMD)

  6. Knowledge of Health Insurance • Learning about insurance benefits and length of coverage • Making calls with parent when there is a question or concern • Applying for other insurance programs (WCRDP) • Re-applying for Social Security benefits • Carrying insurance card(s)
