Our Approach to Discharge Discharge can be defined as “The process whereby a patient is discharged from the acute setting” (NHSLA Risk Management Standards 2010), this pertains to South Tees Hospitals NHS Foundation Trust and patients being discharged from hospital including those discharged to another NHS Trust or care provider.
