Supportedlivingarrangement • Each person may be able to live according his/her wishes. • He/she should be able to live where and with whom he/she wants and feelswell with him • We support people in the transition from the institution to independentlife
Supportedliving arrangement • Rytmusstarted to provideregistered socialservicein 2010 in Karlovy Vary region as a part ofproject oftransformationofresidential care • today we support 45 people in 7 cities • in 2015 westarted in Prague also • todaywesuppport 15 adults
Ourteams • Isdevidedacording to regions (cities) andaccessability • 8casemanagers/support coordinators(7-8 persons) + 30 assistantsindividuallychosenforevery single person + 2 communitysocialworkers

2 targetgroups • Peoplewithdisabilitieswhowanted to leavetheinstitutions • Peoplewithdisabilitieswho do not want to live in theinstitution– preventionofinstitutionalization

Communitywork – part oftheservice • socialinclusion • we created this position two years ago because it was always important but we had never time to do it • wefocus on findingthe role in thecommunityforthe person by definingthegifts and strenghtsofher/him - and matchingthemwithaccesibleplaces in thecommunity
