Streamlined Process Submitting Business On-line • Log into provided agent specific link. • Select product client has chosen. • Follow steps to complete application with client present, sign & upload. • If client not present, Agent skips the signing portion & application is emailed to client. • Client receives email, signs app & returns to agent for agent to complete process. Medical Questions On-line Application • NOTE: • Explanations MUST be required for any questions answered with a “YES” • Explanation to include: Who / What Condition / Diagnosis Dates / Dr. Info • Questions 1-8 pertain to ALL persons on application (applicant - not family history questions) • Question #5 regarding smoker/non-smoker MUST correspond to answer entered at beginning of application process • Question #9 is the family history question 11

Obtaining Client Digital Signature On-line Application • Client Present: Select “Sign Now” • CLIENT MUST BE PRESENT IN PERSON OR VIA SCREEN SHARE TO SELECT THIS OPTION (once client signs, AGENT MUST go to back office to provide agent signature & upload) • Client NOT Present: Select “I do not wish to sign at this time” • Agent MUST then log into back office to send email to client: www.agentusername.virtualinsurance.net/admin Agent will then be required to sign & upload the completed app. Agent MUST then click to Finish Processing Application 14
