  2. Inquiring About Access • Individuals call 800# to inquire about Access • Discusses transportation needs • Provides brief explanation about Access • Sends Access application if wanting to continue or provides alternative transportation information

  3. Receiving Application • Fills out application completely • Basic Personal Info & Emergency Contact • Questions related to disability • Authorization for Release of Information • Healthcare Professional Contact • Responsible Party & Signature • Mails application (self-addressed stamped envelope) • Waits 7 days before calling to book appointment

  4. Application/Evaluation Process • Incoming applications are entered in system • Reviewed for completeness • Contacts applicant (if needed) • Evaluation appointments are booked within 2-3 days of request. • Applicant is evaluated

  5. Application/Evaluation Changes • Will not need to resubmit application if only missing few questions • Driver evaluation will be discontinued

  6. End of Process • Applicant receives determination letter via US Mail within 21 days • Current average is 13 days • 166,776 eligible riders as of 10/31/2017
