  11. CASE STUDY #1 • A 3rd year mechanic was tasked with repairing a small metal part inside the cabin of a groomer. The job was thought to require some welding. This mechanic did some welding in another job years ago. The task also required the mechanic to lay on his back in a small portion of the Cat that did not allow much movement. The welding task took approx. 40 minutes to complete. After the mechanic was satisfied with the completion of the job, he backed the Cat out into the parking area outside of the shop. When he exited the Cat he became dizzy and nauseous. He reported feeling ill to his Supervisor who suggested he go to the Ski Area clinic for evaluation.

  15. Risks???

  16. Identify the risks: Solve for the risks with the “Hierarchy of Controls”. Wha????

  17. Hierarchy of Controls Move the broken part to a welding cage. Most Effective Choose not to weld. Can a new part be ordered? Open all doors and provide air sufficient circulation. All welding performed outside of welding cage must get JHA with Sup. Provide respirator appropriate for HAZMAT exposure. Least Effective

  18. What are the root causes identified? What is the goal of the Action Plan? Who will be responsible for constructing the plan? How will you reach the goal? (This is the meat of your plan): Refer to Heirarchy of Controls When will the Action Plan be complete? (This is your timeline): How will the plan be communicated? (Teamwork!!!) How will you measure success? Incident rate, good communication, etc.
