Sending Transcripts • Fill out transcript request form (found in Guidance Office) and give to your counselor. Your application is not complete without it!! • Self-Reported Transcripts: Some schools, such as The University of Illinois Urbana-Champaign, The University of Iowa, and the University of California system have students “self-report” their classes and grades. A transcript request form should still be given to your Guidance counselor.
