Casenote audit ongoing to review NHS Western Isles performance prior to introducing SEPSIS 6 screening tool • Initially work undertaken on blood cultures sent to lab, not all patients had a SEWS 4> • A further 10 notes audited that met the criteria • NHS Western Isles fall down on time to first dose ranging from 40 minutes – 4 hours • Lactate not routinely done but easy to implement • Blood Cultures done routinely on temp> 38 but not less than 36
Lactate to be performed on every sample • Improve time to first antibiotic dose • Encouraging antibiotic to be given in A/E • Ensure adequate I/V fluids administered • Encorage blood cultures <36
Information Leaflet plus 2 Screening Tools NHS Forth Valley NHS Highland distributed to all wards to raise awareness • During shift work CSN was waiting for someone who fitted the criteria for screening in order to do a PDSA • Small numbers and nobody fitting criteria over the 3 weeks • Next step CSN introduced concept to A/E staff both forms left to be trialed (PDSA)
