  8. １.　境界　boundary • 枕三角 occipital triangle，又称肩胛舌骨肌斜方肌三角，位于胸锁乳突肌后缘、斜方肌前缘与肩胛舌骨肌下腹上缘之间。

  12. 副神经与副神经淋巴结

  13. 副神经损伤 • 副神经脊髓根或外支损伤时，患侧胸锁乳突肌和斜方肌瘫痪，出现头不能向患侧侧屈，肩下垂，提肩无力等临床表现。 • 若在枕三角内损伤副神经，则损伤侧斜方肌瘫痪，出现患侧肩下垂，提肩无力等临床表现。

  23. 静脉角 Venous Angle • 右静脉角有右淋巴管注入。 • 左静脉角有胸导管注入。

  25. （5）锁骨上淋巴结 supraclavicular lymph nodes Supraclavicular lymph nodes • 锁骨上淋巴结supraclavicular lymph nodes：位于颈横血管周围、锁骨上大窝内。 • 魏尔啸（Virchow）淋巴结：即左斜角肌淋巴结 。

  26. 四、颈根部 root of neck • 境界 boundary • 主要内容 main contents • 毗邻 relation

  27. 1. 境界 boundary • 颈根部是指颈部与胸部之间的接壤区，由进出胸廓上口的诸结构占据。 • 境界为：前界为胸骨柄，后界为第1胸椎体，两侧为第1肋。

  28. 颈根部结构以中心标志为前斜角肌分为： • 前斜角肌前内侧的纵行结构，主要往来于颈、胸之间，脏器（气管、食管、肺尖和胸膜顶）血管（颈总动脉、颈内静脉、胸导管）、神经（迷走神经、膈神经、颈交感干）等； • 前斜角肌前、后和外侧的横行结构，主要往来于颈、胸与上肢之间，如血管（锁骨下动、静脉）、神经（臂丛）等。

  31. (4) 锁骨下静脉subclavian vein • 行程 • 与周围筋膜的愈着情况：锁骨下静脉壁与第1肋、锁骨下肌和前斜角肌的筋膜相连，故伤后易导致空气栓塞。 • 临床应用：行锁骨下静脉穿刺插管术，进行长期输液、心导管插管及中心静脉压测定等。

  32. （5）胸导管 Thoracic Duct • 位置和开口部位：胸导管沿食管左侧出胸腔上口至颈根部，达第7颈椎高度，向左呈弓状经颈动脉鞘后方，跨过胸膜顶、椎动脉、颈交感干、甲状颈干、膈神经、锁骨下动脉，注入左静脉角。 • 临床应用：胸导管逆行造影、胸导管引流术、胸导管颈内静脉吻合术等。

  33. （6）迷走神经 Vagus Nerve • 迷走神经：在左颈总动脉与左颈内静脉之间下行入胸腔。 • 右喉返神经：在右锁骨下动脉第1段前面起自右迷走神经，经该动脉的下面、后面向上行向右气管食管旁沟内。

  34. （7）膈神经 • 组成：由第3~5颈神经前支组成。 • 行程：被覆着椎前筋膜，沿前斜角肌浅面斜向内下方，经锁骨下动、静脉之间入胸腔。

  35. 境界：外侧界为前斜角肌，内侧界为颈长肌，下界为锁骨下动脉，尖为第6颈椎横突前结节。境界：外侧界为前斜角肌，内侧界为颈长肌，下界为锁骨下动脉，尖为第6颈椎横突前结节。 Cervical sympathetic trunk Inferior thyroid artery （8）椎动脉三角 vertebral arterial triangle ﹡ ﹡ ﹡ ﹡ ﹡ • 内容：椎动脉、椎静脉、甲状腺下动脉、颈交感干及颈胸神经节等。 • 三角的前方的结构：有颈动脉鞘及其内容、膈神经和胸导管等。

  39. 2.颈部淋巴结The lymph node of the neck • 颈部淋巴结沿静脉纵向排列，或位于消化道和呼吸道周围。 • 分为： • 颈前淋巴结：颈前浅淋巴结和颈前深淋巴结。 • 颈外侧淋巴结：颈外侧浅淋巴结和颈外侧深淋巴结 • 收集头颈部的淋巴，经颈干汇入左右淋巴导管。

  42. 颈内静脉肩胛舌骨肌淋巴结 jugulomohyoid lymph node：引流舌尖的淋巴。 颈内静脉二腹肌淋巴结jugulodigastric lymph node ：引流鼻咽部、舌根和腭扁桃体的淋巴。

  43. 咽后淋巴结retropharyngeal lymph node：位于咽后壁和椎前筋膜之间，引流鼻腔后部、鼻旁窦、鼻咽部和喉咽部的淋巴，输出淋巴管注入颈外侧上深淋巴结。

  46. 2.松解锁骨：在胸锁关节处离断锁骨与胸骨和第1肋软骨的连结结构，沿锁骨下面切断锁骨下肌，将锁骨向上牵拉。2.松解锁骨：在胸锁关节处离断锁骨与胸骨和第1肋软骨的连结结构，沿锁骨下面切断锁骨下肌，将锁骨向上牵拉。

  47. 3.解剖锁骨下静脉及其属支 • 颈外静脉 • 颈横静脉 观察椎静脉的注入情况

  48. 4.解剖锁骨下动脉及其分支 • 在前斜角肌内侧寻找锁骨下动脉第1段的分支：甲状颈干及其分支（甲状腺下动脉）、椎动脉、胸廓内动脉 5.解剖胸导管

  49. 6.解剖迷走神经 • 在颈内静脉与颈总动脉之间后方寻找迷走神经，并向下追踪至胸廓上口，观察其行程。 • 在右锁骨下动脉的前下方寻找由右迷走神经发出的右喉返神经，并向上追踪之。 7.解剖膈神经 • 在前斜角肌浅面、椎前筋膜的深面寻找。并向下追踪至胸廓上口，观察其行程。

  50. 8.解剖臂丛和锁骨下动脉第3段 • 在前、中斜角肌之间寻找臂丛及锁骨下动脉，并追踪至腋窝。 9.解剖颈交感干
