Fallen Barite Tanks What Happened:While one Barite Tank was being loaded with the second load (40,000 lbs per load) and the other Tank was empty, the Tanks became off balance and fell toward the suction pit where two employees were mixing a slug. Investigation revealed an unstable sandy soil; unlevel ground, and unsecured tanks to the stand were all contributing factors in this near miss incident.

Corrective Actions: This incident could have been easily prevented if some level matting had been placed under the stand; the tanks secured with bolts to the stand and loading procedures were equally distributed into both tanks.
