General Practice Queensland (GPQ) • General Practice Queensland is a state based organisation that provides strategic leadership, advocacy and representation for 17 divisions of general practice • GPQ established in 1997 • Private MSOAP fundholder since 2000. • GPQ works in conjunction with Qld Health MSOAP team, who is the other Qld MSOAP fundholder.
GPQ MSOAP Services • MSOAP budget: • - 2009 - 2010 service delivery $2,155,242.40 • - 2010 - 2011 service delivery $1,979,325.00 • 2009 - 2010 • provides 116 individual services • contracts 36 service providers • visits 54 locations - from Torres Strait in the north, Cunnamulla in the south, and Mount Isa in the west.
Closing the Gap • Qld Total • 3808 patients benefited from private outreach specialists • 640 or 16.8% of these patients identified as Aboriginal or Torres Strait Islander origin
Services in Cape York • Cape York Welfare Reform Communities • From July - December 2009, 36 of the 206 visits have been delivered throughout Cape York • From 1/07/09 – 30/11/09 • 558 patients benefited from private outreach specialists • 305 or 64.5% of these patients identified as Aboriginal or Torres Strait Islander origin • Highlights • ENT Services delivered to Aurukun and Hope Vale • ENT service delivered to Cooktown
Closing the Gap • Within the Indigenous Health National Partnership Agreement, five priority areas in relation to chronic disease have been identified: • Tackling smoking ($161 million) • Primary health care services that delivers ($474 million) • Fixing the gaps and improving patient journey ($171million) • Providing healthy transition to adulthood • Making Indigenous health services everyone's business
MSOAPIndigenous Chronic Disease Measure (ICD) • MSOAP ICD: expansion of the existing MSOAP - linked to priority area “primary health services that deliver”. • Funding: $54.4 million over four years allocated to expand MSOAP - ICD. • Auspicing agencies: 9 across Australia, including GPQ, QHealth, other State Health Departments, and Health Workforce Agencies.
MSOAP Indigenous Chronic Disease Measure (ICD) • Implementation: Service delivery commencing 1 July 2010 - to include medical specialists, GPs, allied health. Pilot services prior to July 2010. • Target: Rural and remote Indigenous communities with a high prevalence of complex and chronic disease. • Chronic conditions under MSOAP - ICD are: • Diabetes • Cardiovascular disease • Chronic respiratory disease • Chronic renal (kidney) disease • Cancer
GPQ - QH • Coordinated approach • Planning • Service implementation • Delivery of service • Review and reporting • Outcomes • Assessing Cape York needs to plan services annually
