Audit Report: Intruder • Finding #1: Auditor entered the facility at 9:56 a.m. by walking through the reception area, through the clinic and into the hallway. Auditor continued to circulate the facility for seven (7) minutes before concluding the Intruder Drill and self reporting to the facility’s visitor registration site.
Audit Report: Intruder • Finding #2: After identifying themselves at the facility’s visitor registration site, auditors were not asked to produce identification, register, or to sign-in. Auditors were allowed to begin their interior and exterior surveys without obtaining visitor badges. After being on campus for approximately 30 minutes, auditors were asked to properly sign-in and register.
Audit Report: Interior Security Finding #4: Auditor found offices in both administrative suites to be unlocked during times when they were unoccupied. In one office, auditor found a file cabinet containing confidential and sensitive student information unlocked. Auditor was able to access these offices by entering from the hallway through the rear entrance.
Audit Report: Interior Security Finding #11: Staff members, based on information provided during interviews, were unfamiliar with lockdown procedures for common areas such as the library and cafeteria.
AuditAudit Report: Safety & Security Leadership Report: Finding #6: Auditors found no formal resource list identifying staff with certifications, licenses and specialized skills. Finding #7: Based on verbal reports from staff, conducting table top exercises is not a formalized, on-going occurrence at this facility.
AuditAudit Report: Safety & Security Leadership Report: Finding #10: Although a facility based Emergency Response Team (Campus Safe & Secure Action Team) has been established, it was reported that team members meet only once a semester. Administrators further acknowledged that Campus Safe and Secure Action Team members have not been identified in writing and clearly communicated to staff members.
Audit Report: Campus • Intruder Finding #1: Unauthorized access was granted to auditor by a facility staff member.
Audit Report: Exterior Security • Finding #3: Near Exterior Exit #5, Auditor observed a brick and several large rocks that could be used as projectiles by students during horseplay or by vandals to cause property damage.
Audit Report: Security Leadership Finding: Although the receptionist showed auditor a visitor log and stated that visitors are required to sign-in, auditor was not requested to sign the log. Without a consistent sign-in procedure, it would be difficult to provide emergency responders an accurate accounting of visitors in the facility.
