  1. Severity 4 & 5 observations May ’2010 West Region

  2. Severity 5- Damaged rope Ladder was being used for access to roof (Sun pharma) _West Action Taken: Rope Ladder removed from site & Scaffolding arranged .

  3. Severity 5- Sheeters were ascending & descending w/o anchoring lanyard (Sun Pharma) _West Action Taken: Stopped activity &. Explained importance of anchoring ,Training given to all workers .

  4. Severity 4- Oxygen & L.P.G. Cylinder without Flash Back Arrestor, Fire Extinguisher, & Barricading & trolley. (Jindal Rail) _West Action Taken: Work was stopped and asked builder to provide flash back arrestor and trolley for cylinder

  5. Severity 4- Life Line found damage.(Jindal Rail) _West Action Taken: Life line replaced.

  6. Before After Severity 4- Compressor without Guard , Barricading, Fire Extinguisher & Emergency cut off Switch. (Jindal Rail) _West Action Taken: Compressor replaced with New one. .

  7. Severity 4- Painter Helper without Life Line. (Jindal Rail) _West Action Taken: Work Stopped & Life Line Provided. .

  8. Before After Severity 5- Workers found sitting on purlin for secondary work. (Jindal Rail) _West Action Taken: Stopped work, Provided life line with Sag angle platform. .

  9. Severity 4- Extra Fiber & MR-24 Sheets found loose on Roof. (Arshiya) _West Action Taken: Extra Sheet removed from Roof.

  10. Severity 5- Sheeters working with out full body harness & life line at Eve End. (Arshiya)_West Action Taken: Work was stopped and full body safety harness provided and life line .

  11. Severity 5- Hydra safety Latch & sling found damage. (Grasim Wada)_West Action Taken: Safety latch repaired & damaged sling discarded. .

  12. Severity 5- Electrical Board & wire laying on ground w/o proper Stand. (KD. Logistics,)_West Action Taken: Electrical Board rectified. .

  13. Severity 5- Riggers Sitting & anchoring safety belt on girt (Unique Delta) _West Action Taken: Work Stopped and asked builder to mobilize proper scaffolding . .

  14. Severity 4- Rope ladders were being used as access & also in damaged condition (Unique Delta) _West Action Taken: Work Stopped and all rope ladders were removed from site, Issued warning to builder & asked builder to mobilize Mobile scaffold for same work (Action taken). .

  15. Severity 4- Scaffold Wheel found w/o wheel Lock. (Unique Delta) _West Action Taken: Work Stopped and scaffolding with wheel lock arrangement provided

  16. Severity 4- Compressor Electrical Box found open.(Unique Delta) _West Action Taken: Work Stopped and asked builder to rectify .

  17. Severity 4- Main Board RRCB found Bypass , naked wires, Starter without cover (Unique Delta) _West Action Taken: Work Stopped and compliance done .

  18. Severity 5- Non standard Man basket found at site (Unique Delta) _West Action Taken: Removed the Man basket from site , Builder to mobilize boom lift. .

  24. BEFORE After Severity 4- : Primary member erected without lifeline post (Perlos, Chennai) _ TN Action Taken: Stopped work, Necessary fall protection arrangement provided .

  25. BEFORE NOW Severity 5- : Scaffolding provided for tie beam connection not having mid rail and top rail etc., (Perlos, Chennai) _ TN Action Taken: Stopped work, Scaffolding height increased, boomlift mobilised .

  26. BEFORE NOW Severity 5- : Column erected without bracing and steel guy ropes not used(Perlos, Chennai) _ TN Action Taken: Stopped work proper bracing provided for erected column, Asked to use only steel guy ropes .

  27. Severity 5- : Workers found working below suspended loads (Perlos, Chennai)_TN Action Taken : Stopped the activity and conducted training for all workers .

  28. Severity 4- : : Unsafe loading of primary member received at site (Perlos, Chennai)_TN Action Taken : Issue has been highlighted for necessary compliance to logistics team

  35. Severity 4- Roof access not proper erected and not anchored (Grasim) _ AP Action Taken : Worked stopped asked builder to provide proper access

  36. Severity 4- Worker found coming down the roof in a unsafe manner(Grasim) _ AP Action Taken : work stopped ,working at height training conducted for all workers

  37. Severity 4-Lifeline not tied on the 51st rafter (Ultra Tech)AP Action Taken: : Work stopped , Instructed Builder supervisor to provide the lifeline (Done)

  44. Before After Severity 5-: Scaffolding base was kept in loose soil. (M/s Bengal Shristi,)_East Action Taken: Hard surface(soling plate)with compacted soil provided under the base of scaffold.

  45. Severity 5-: Open cable joints (M/s Maa Amba, Kolkata)_East Action Taken: Industrial socket(Male- Female) provided .

  46. Severity 4-: Workers were getting down from rafter through brace rod.(Rozer plant, Raipur.)_East Action Taken: Work stopped immediately & provided roof access.

  47. Severity 5-: Improper Scaffolding -no access and no work platform (Rozer plant, Raipur.)_East Action Taken: Activity stopped and corrected
