Hydro-implantation of Rigid IOLs in Manual Phaco (SICS) – A Safety and Efficacy Study ^ Dr. Suvarna Bhagde Sadguru Netra Chikitsalaya, Chitrakoot.
Introduction • Classically, in a cataract surgery, IOL is implanted after the capsular bag is inflated and anterior chamber is formed with viscoelastic. • Problems with Viscoelastic– • Takes time to be removed completely from the eye • Often inadvertently left behind causing • Post-operative IOP spikes • Pseudo-anterior uveitis. • To circumvent these problems, we implanted rigid PMMA IOLs under a continuous irrigation of RL from the side-port in manual phaco surgery.
Aim To study safety and efficacy of Hydro-implantation of rigid IOLs in manual phaco (SICS).
Material and Method Nature of Study : Prospective, Non-controlled, Interventional Cohort Study Setting : SadguruNetraChikitsalaya, Chitrakoot Duration : February 2010 Inclusion : 130 eyes of 130 consecutive patients Exclusion : Patients with concomitant Glaucoma Previous intra-ocular surgery
Results 130 eyes of 130 patients Average Age : 58.6 years (Range : 35 to 90 years)
Types of Cataract Two cases of Hypermature Cataract showed Phacodonesis.
Discussion • There are no published studies on only Hydro-implantation of IOL in SICS surgery. Work has been done on Blumenthal Technique wherein also the IOL is implanted in visco-free setting by some workers1,2 • Hydro-implantation of IOL has been tried in Phacoemulsification3,4. • Following advantages of Hydro-implantation of IOL are seen: • No OVD-induced intraocular pressure elevation postoperatively • No risk of early capsular bag distension syndrome • Reduced post-operative anterior uveitis. • Improvements in surgical efficiency • Reduced surgical cost • Malik KP, Goel R. Conclusion Thanks Hydro-implantation of IOL is safe in experienced hands with minimal complications and acceptable results.
