5 Goals of the NC ADRC Pilot Programs • Develop ADRC Programs in 2 Counties (One Urban and One Rural Model) That Will Be Visible and Trusted by Both Older Adults and Adults with Disabilities • Empower Older Adults, Adults with Disabilities, Caregivers and Other Consumers to Make Informed Choices about Their Care and Identifed Needs (Consumer Directed) • NC care Link (up an running Sept 2007) –website • http://www.nccarelink.gov/ • Seamless Streamlined Access to Public Programs (use of a uniform screening tool/potential to share client data among data contributors using NC care Link thru client tracking software (under development ) • Sustainability - Moving toward Rebalancing LTC -
