Last 4 digits of sponsor’s Social Security Number Patient Name Number of units (tablets, boxes, etc.) dispensed Medication Name- GENERIC name (BRAND name). Date your provider wrote the prescription. Prescription will expire 12 months from this date. Controlled substances expire in 6 months. Number of refills left. First number is the number of refills left. Second number is the total number of refills authorized by your doctor Directions for taking the medication
