Laparoscopic Redo Surgery • Success influenced by conduct of index procedure • Laparoscopic Index Procedure • Open Index Procedure • As a general rule re-operative surgery can be attempted laparoscopically
Lap IPAA • Pouch surgery often performed in three phases • Emergent, urgent or elective total colectomy • Proctectomy with ileal pouch anal anastomosis • Closure of ileostomy
Laparoscopic IPAA • If index procedure done laparoscopically consider Pfannenstiel for specimen retrieval and Pouch creation • All adhesions need to be divided to ensure adequate pouch length • If index procedure performed lap then proctectomy should prove feasible
Lap Hartmann’s Closure • Tips • Take down the stoma first • Assess adhesions through stoma aperture • Place single 5mm trocar • Assess adhesion laparoscopically • Make a quick decision • Intra-loop adhesion and significant pelvic adhesions can prove too much
Redo Laparoscopy for Recurrent CD • Fistulizing CD predictor of failure • Index surgery done open – no reports • Conversion rates consistently high
