For the next 20 minutes… • Understand patient self management • Watch and assess a healthcare provider’s behaviour of patient behaviour change • Review aspects of behaviour change process • Review behaviour changes interview techniques • Leave time for discussion
What is patient self management ? People with chronic health conditions often need education, coaching and other interventions to help them gain the confidence, knowledge, skills and motivation to manage the physical, social and emotional impacts of the disease.
Increasing importance • people are often living with more than one chronic condition • Research shows the impact of self management skills in • disease control • stress management • reduced stress upon primary care
Outcomes of Self Management • Research shows a 90 billion a year cost on productivity and health care costs • Patients who are effective in self management have learned the skills • has reduced their reliance on primary care

Surgeons comments • Should she have “the power”? • Aware that she may need skills • needs to find strategies to promote behavioural change • Feels defeated- risk of burnout • I do not have the power to convince • Maybe I am going about it the wrong way • I can’t find a way to influence patients • I bring it up less and less because I do not get anywhere
The cycle of self management support in Primary Health Care • It involves building a relationship over time • Three stages: Before the Visit During the Visit After the Visit
5 A’s of Self managment • Assess • Advise • Agree • Assist • Arrange
The healthcare provider is the intervention • Key elements 1 Assess before advise skills of open ended enquiry, assess agenda, assess patient conviction and convidence 2 Build rapport 3 Tailor the plan 4 Follow up
Recommendation 1 • Create an integrated, system –wide approach to self management support • Current efforts to promote, deliver and evaluate self management support are often fragmented
Recommendation 2 • Enable primary health care providers and those who work with patients deliver self management support as a part of routine care
Recommendation 3 • Broaden and deepen efforts to reach more Canadians who need self management supports
Recommendation 4 • Engage patients and informal caregivers as a key part of any systematic approach
