3. Blood Tests Patients do not have to attend the surgery for their blood tests. You may find it more convenient to attend Ipswich or West Suffolk Hospital. Opening times are as follows or can be accessed via the hospital website. Remember to take your forms with you.

4. Blood tests Ipswich Hospital 01473 703721 Some appointments can be booked in advance on the above number or attend the open access blood test clinic from 7.30am to 5.45pm Monday to Friday. West Suffolk Hospital 7.30am to 7pm Monday to Friday and 9am – midday on Saturday

6. Seasonal Flu Clinics • Free vaccinations if you are; • Over 65 • A Carer • Pregnant • Or suffer from; • Heart problems or a chest complaint • Asthma requiring regular inhalers • Kidney disease or diabetes • Lowered immunity or are a stroke victim

7. SEASONAL FLU CLINICS Saturday 4th October 2014 Saturday 25th October 2014 Please note that these sessions are for patients eligible for the seasonal flu vaccination and have booked an appointment in advance only.

9. Flu Clinics • This year all children aged 2, 3 and 4 years old will be offered a flu vaccine administered via a nasal spray • Children in this category will receive a letter inviting them to a special clinic • Look out for your letter in September

10. Shingles vaccine….. • New shingles vaccination • Available NOW • Very strict age criteria apply • Initially only for those who were aged 70, 78 or 79 on the 1st of September 2014 • Please book an appointment with the receptionist

11. Physiotherapy – self referral • Patients are able to refer themselves for physiotherapy advice either online or by phone • Initially advice and exercises • Onward referral if necessary • No need to see your GP! • Pick up a card from reception

12. Physiotherapy “Refer yourself to Physiotherapy” Via www.ahpsuffolk.co.uk Or telephone 01473 770066 This service is run by Allied Health Professionals under contract with the NHS

13. Saturday Morning Clinics From September there will be a nurse or HCA led clinic as well as a GP clinic each Saturday. All appointments must be booked in advance. We regret that “walk in” patients are not able to be seen during these surgery times. Thank you.

15. Tuesday 7am clinic From September we will no longer be running the early Tuesday morning GP clinic or the early Blood clinic from 7-8am However we will be offering extended hours on a Saturday morning instead. Blood tests are available each week day morning from 8.20am but are also available at walk in clinics at Ipswich and West Suffolk Hospitals.

16. Dr Adam Wankowski Dr Wankowski has been overwhelmed by the kindness and generosity of his patients on his retirement. Having founded the Combs Ford Surgery and following 25 years serving the people of Stowmarket it is time for a change and a rest! Dr Wankowski’s patients have now been advised of their new GP.

19. Surgery closure- Staff Training The Practice will be closed for essential staff training on the following dates • Thursday 11th of September 1-4pm • Wednesday 15th October 1-4pm • Thursday 13th November • Wednesday 10th December

20. Consent • Informed implied or expressed consent will be obtained before any procedure or examination is carried out. • An explanation will be given to the patient sufficient to understand the procedure. • Should the procedure carry a risk that the patient is likely to consider as being substantial, expressed consent (written or verbal) will be obtained. Copies of the consent policy are available on request.

21. Zero Tolerance The Practice operates a zero tolerance policy • we will not hesitate to remove patients from  our list who: • are violent or abusive to any Practice member. • use or condone threatening or violent behaviour. • cause damage/steal from the Practice's premises/staff/patients. • obtain drugs and/or medical services fraudulently. We ask you to treat your GPs and their staff courteously at all times.
