CAO-AM Assessment Results • Total Chemical Sample = 270 • Total Errors = 133 • Percent Error = 49% • Percent Accurate = 51% Breakdown of Errors • Missing Barcodes = 2 • Chemicals present in the building but not listed in ChemLog = 31 • Chemicals listed in ChemLog but not found during the facility tour = 98 Possible Causes for the Error • 72 of the 98 not-found chemicals from our original list were listed in ChemLog under Obie Gillespie as owner, but according to Doris Ford, the chemicals were never in the room. In 7/21/04 Doris Ford submitted a message requesting that they be removed from the ChemLog list, explaining that the chemicals were never in the room. In 10/03/07 Obie Gillespie resubmitted the request. They are still on ChemLog.

  6. Lessons Learned • Allow ChemLog to inform Chemical Owners that they have chemicals and that a chemical inventory is due. • Provide Barcode Readers that are allowed at TA-55. • Require Chemical Owner be present during an Assessment.

HMPP • TA-55 has had a Hazardous Material Protection Program (HMPP) since 1999, per DOE Order 5480.23, Nuclear Safety Analysis Reports. • Institutional expectations of ISD101-14, Chemical Management and the Technical Safety Requirements of Section 5.6.8 of TA-55-PED-108-01, TA-55 Technical Safety Requirements (TSRs) are implemented through this program.

Key Elements of HMPP • Hazardous Material Working Team(HMWT) • Consists of Chemical Hygiene Officers and ESH Subject Matter Experts • Meets monthly to address hazardous material issues • Coordinates annual chemical inventory • Reviews hazardous material incidents • Recommends measures to minimize hazardous material incidents • Responds to audit findings and concerns related to hazardous material incidents at TA55 Division facilities

Key Elements of HMPP • Incorporation of the elements of Integrated Safety Management • Bioassay and medical monitoring • Workplace air sampling and record keeping • Instrumentation and calibration • Hazard evaluation and control • Provisions for specific materials and hazards, e.g., perchloric acid, explosives, and toxic gases

  10. Key Elements of HMPP • Chemical Management Assessments • Each of the elements on the previous slide is assessed, at least annually • The results of these assessments are documented in LIMTS
