Referrals HCCC created a referral department to better facilitate patient referrals. • Medical: 10,982 (6,410 Tests; 4,572 Specialists) • Dental: 6,628
Patient Assistance Program • Patients – 1,041 • Number of Prescriptions – 53,107 A full time nurse helps patients receive free medication through enrollment and monitoring of the Pharmaceutical Indigent Programs. HCCC staff coordinate with patients and pharmaceutical companies to help patients receive the medications they need.
HCCC was one of the first clinics to utilize the State MCIR registry. Our motto: Never miss an opportunity. Childhood Immunizations: 6,711 Lab Tests: • In-house: 11,880 • Sent-out: 16,333 Flu Vaccines: 2,734 HCCC provides a walk-in flu clinic for fast and easy flu shots.
School Based Health Program: 1176 Kids Served HCCC staff are located in Steele, Bunker and Muskegon Heights Middle schools. The program offers: • health, fitness, and nutrition education • health screenings • case management services • after school programs Dental in Schools: 84 Kids Served A dentist, hygienist, and assistant travel to area schools to provide dental exams and sealants to elementary and middle school children.
Breast & Cervical Cancer Program Komen Grant: The Breast and Cervical Cancer Screening program is a service provided for women who are 40 years and older, uninsured, and who meet the income guidelines. This service provides women with a clinical breast exam, mammogram, pelvic exam, and pap smear free of charge. • 184 women seen • 21 women received emergency funding. • 209 mammograms were paid for.
Grandparents Raising Grandchildren: Chronic Disease Classes: A 6-week, 2 ½ hour once per week workshop that builds confidence for managing health, staying active and enjoying life. • 37 participants, 5 classes • 80% lowered their A1C within a 6 month time period You are not alone, a program that provides support groups and education for grandparents raising their grandchildren. • 57 grandparents participated
Student/Preceptorship: What a great experience for students and staff. • U of M Dental Students: 31 • Physician Assistants: 5 • Medical Assistants: 2 AmeriCorp Program: HCCC has 2 AmeriCorp volunteers – one works with School Based Health Program and one concentrates health literacy, reading and getting books to give away to patients. • Over 22,000 books were received
HCCC Healthy LivingProgram (Season 1): Casual For A Cause: • 45 participants • $2,459.00 paid out to staff HCCC staff pays $2 to wear jeans on Friday. All the money goes back to community agencies (Love Inc., Rescue Mission, MAP, Meals on Wheels, etc.). • $2,300 donated to community in 2007 • $19,870 since 1997
