Background – PA adopts the SISTM The PA DHS Office of Developmental Programs, together with stakeholders, chose the Supports Intensity ScaleTM(SISTM) to be the standardized needs assessment in Pennsylvania for individuals with intellectual disabilities. The SISTM was developed by the American Association on Intellectual and Developmental Disabilities (AAIDD) in 2004 and is used in several states. The SISTM was chosen so that needs of all individuals aged 16-72 who are entering or enrolled in the PA ID Waivers will be assessed consistently and equitably across the Commonwealth. Beginning in January 2018, ODP implemented a Fee Schedule for Consolidated Residential services utilizing the SISTM and PA Plus (9 additional assessment questions).

Objectives of the Learning Collaborative • A Learning Collaborative is a systematic approach to process improvement. • Organizations can test and implement system change and measure the impact. • The Collaborative will share their experiences to accelerate learning and broader implementation of best practices. • Learning occurs through active engagement among peers, to search for understanding, meaning, or solutions. The group can create a product of their learning. LEAP Learning Collaborative: • Implement and test tools that were developed to validate the need for exceptions to the Residential Fee Schedule. • Pilot communication, training and technical assistance structures involving multiple Counties. • Utilize expertise available at the County level to better assist the State with process implementation and improvement. • Plan>Do>Check>Act – continue to redesign and modify • Explore opportunities to apply lessons learned to other areas of system change.
