VALIDATING CONCERNS • Schedule appointment with child’s Pediatrician Ask for a referral to a specialist who can perform a more in-depth evaluation. • Developmental Pediatrician • Child Neurologists • Child Psychologists or Psychiatrists
VALIDATING CONCERNS Birth to 36-months: • Point of Entry: San Andreas Regional Center (SARC) • Schedule an Intake Interview • Local Education Agency(LEA) & SARC will provide assessment • Eligibility for Early Start will be determined Over 36-months to 21-years: • Point of Entry: School District Provide a written request for assessment School District will provide assessment Individualized Education Plan meeting will be scheduled Contact Special Kids Crusade for assistance.
GETTING SERVICES-Early Intervention Birth to 36-months: • San Andreas Regional Center Consultation with Family Services Coordinator Early Intervention Team recommends services* Individual Family Service Plan (IFSP) developed Transition assistance into preschool services at 36-months *Early Start Services may include Assistive Technology, Family Training & Home Visits, Health & Medical Services, Respite, Occupational & Physical Therapy, Speech & Language Service.
GETTING SERVICES-36-months to 21-years • Special Education through School District or MCOE • Individualized Education Plan (IEP) meeting will be scheduled following District performed assessment • IEP will list all services to be provided • Parents should be well-informed prior to IEP meeting • San Andreas Regional Center • Schedule an Intake Interview • Intake Team will gather information from School District, physicians, psychologists & other professionals • Eligibility for services will be determined (only services not available through School District)
