Assistance for the Uninsured Uninsured Patients Free Medicines Alternate Funding Assistance Pfizer RxPathways Savings Card Benefits • Pfizer RxPathways Savings Card helps patients get savings on Pfizer medicines through participating pharmacies • Varies by product and income level • For primary care medicines, Pfizer refers patients to www.healthcare.gov • For oncology and specialty medicines, Pfizer will assist with eligibility and process for Medicaid or Part D subsidies • Patients must: • Live in the US, US Virgin Islands, or Puerto Rico • Not have prescription coverage • Meet income guidelines and receive treatment from a licensed health care provider on an outpatient basis Eligibility
Assistance for the Underinsured Underinsured Patients Co-Pay Help Alternate Funding Free Medicines Benefits • Patients who participate in federal or state health programs, such as Medicare or Medicaid, may not be eligible for co-pay help • These patients can call Pfizer RxPathways to learn more about alternate funding options for their prescription medicine • If no alternate funding is found, patients can apply for Pfizer RxPathways • Eligible underinsured patients with private or commercial insurance can either apply for co-pay assistance through Pfizer RxPathways for certain medicines or be directed to other Pfizer co-pay programs that may be able to help • If co-pay help and alternate funding are not available, eligible underinsured patients may be able to get their medicines for free through Pfizer RxPathways • Patients must: • Live in the US, US Virgin Islands, or Puerto Rico • Meet income guidelines and have insurance, but not be able to afford their medicines due to financial hardship Eligibility
Assistance for the Insured Insured Patients Appeals Process Information Benefits Verification Prior Authorization Information Specialty Pharmacy Referral Benefits • Information on appeals process if a claim is underpaid or denied • Access to select Pfizer medicines through select specialty pharmacies • Assist patients in investigating prior authorization requirements and explaining the process • Counselors will review patients’ benefits and explain coverage options and policies Eligibility
