Evidence Reviews • Only send evidence if letter accompanying statement of deficiencies instructs to do so. • If an onsite revisit is done instead -- in conjunction with another survey, & tags are cleared or rewritten, no need to send in evidence, if have not done so already. • May receive an onsite revisit, even if letter indicates Evidence Review.
Evidence Reviews • Send evidence by due date in letter or earlier. • Evidence sent earlier than due date: > if evidence is complete, please indicate so, in order that it may be scanned to field offices to conduct evidence review as soon as possible. > if partial evidence is sent, please indicate that we should be expecting the remainder of the evidence at a later date.
Evidence Reviews • Mailed evidence should be free of staples, clips, binders, dividers, sticky notes, etc. Evidence Reviews are primarily scanned to field offices throughout the state, & these items must be removed prior to scanning. • Evidence should be in order of deficiencies. • Evidence should be separate & not intermingled with the Plan of Correction(POC).
Evidence Reviews • Evidence should be one-sided onlyto alleviate time spent on unnecessary copying so it can scanned to field offices in a more timely manner. • Evidence points should not be highlighteddue to dark highlighter will block out the area when scanned, & fluorescent yellow will not be visible at all when scanned. Just underline & circle, if necessary.
Evidence Reviews • Evidence should include documents which prove your POC is in working order. This is the same documentation that surveyors may pull while conducting an onsite revisit. • Evidence should include residents specifically noted on statement of deficiencies plus a couple of more similar residents. • Evidence should include beginning of supervision & monitoring.
