Family Centered Care (Dependency) • Children are dependent upon parents/caregivers • PCMH educates and encourages shared family decision-making. • Children are dependent upon others economically and are more vulnerable to poor health outcomes
Positive Outcomes • Continuity of care over time • More supportive transition to adult care • Increased access to preventive care (well visits) • Reduced cost of care
